Instructions for filling out the Tofes Yarok when applying for your Israeli driver’s license.
When applying for your Israeli driver’s license you are required to fill out the ‘Tofes Yarok’ (the green form, as it once was). These days of course you can do it all online (where the form is no longer green – but the name remains).
The form is accessed via the Ministry of Transport’s website. We have provided a translation of the online form which you can use as a guide when filling in the required information.
As you will notice, the second line of information is the license category you are applying for (A, B, C, D – motor cycle, vehicle up to 4 tons, tractor etc.) Here is a list of those different categories and types of driver’s licenses
You will notice that just above the PROCEED button there are 2 checkboxes:
- I confirm that the mobile number and email address provided will be used to send messages and updates regarding the status of your application (This is a mandatory checkbox)
- I willingly give consent to use other updated contact details, provided by me, for any further communication between myself and the Ministry of Transport (not mandatory)
In the Telephone Verification section (see below) you are required to enter your mobile telephone number, and then re-enter it, for verification purposes. A text message with the verification code will be sent to you via SMS or via Voice messaging as per your selected preference.
Upon receiving the verification code, enter it in the space provided.
Upon clicking the אישור (CONFIRM) button, you will be directed to the health declaration section.
The column on the left-hand side confirms NO – לא and the column on right-hand side confirms YES – בן to your medical status. Please make the appropriate selection.
The next step in the application process is signing the application and declaring that the information provided, is accurate. All fields are mandatory.
Once you have completed the online application, you will receive a confirmation SMS from the Ministry of Transport /Licensing Department with instructions for the next steps – the eye test and a photograph.
